Abstract

<p>The case involves a 55-year-old male patient who presented with cervical pain and was subsequently transferred to our emergency department by the previous healthcare facility the following day. Laboratory blood tests revealed elevated markers of inflammation and decreased platelet count. A computed tomography (CT) scan demonstrated swelling of the hip joint capsule and the presence of gas within the surrounding muscles. Gas was also observed within the bone marrow of the femur and pelvis. Due to the patient’s shock state, he was transferred to our emergency and critical care center. A contrast-enhanced CT examination of the trunk revealed the presence of fluid and emphysematous lesions in the left hip joint, emphysematous lesions within the bone marrow of the pelvis and femur, as well as emphysematous lesions within the intermuscular and intramuscular layers surrounding the left femur. The patient was diagnosed with emphysematous osteomyelitis. Initial management prioritized internal medicine treatment to stabilize the patient’s respiratory and circulatory dynamics, followed by multiple surgical procedures for abscess drainage. Emphysematous osteomyelitis is a condition that is rarely reported domestically. However, on the other hand, it is a disease associated with a high mortality rate. We present this report as a testament to our ability to save the patient’s life, despite the inherent challenges faced during the treatment of emphysematous osteomyelitis.</p>
