Abstract

A 64-year-old female noticed a tumor on the left thigh 3 years before presenting to our hospital. Physical examinations revealed a 13×10 cm elastic soft subcutaneous tumor on the inner thigh. Magnetic resonance imaging showed a mass of high signal intensity resembling adipose tissue on both T1- and T2-weighted images. No nodular or thick septal structures were identified within the mass, and no high-signal areas were observed within the tumor on fat suppression images. Under the clinical diagnosis of lipoma, surgical resection was performed. Histopathological examination of the excised specimen revealed that the tumor is composed of mature fat cells with a significant variation in size, a small number of atypical stromal cells, and atypical lipoblasts. A histopathological diagnosis of atypical lipomatous tumor/well-differentiated liposarcoma was established. Since marginal resection was performed, the patient did not require additional surgery during follow-up. There has been no evidence of recurrence 19 months postoperatively.[Skin Cancer (Japan) 2023 ; 38 : 223-227]
