目的：「高齢者の過去の背景に関する情報共有ツール」の活用可能性を検証する。方法：介護医療院に勤務する看護師５名が，高齢者の過去の背景に関する情報共有ツール（以下，ツール）を２か月間使用し，その使用前後にフォーカスグループインタビューを実施した。その後，活用可能性に対するツール使用後の看護師の認識やツールを用いて展開する個別性のある看護の様相等について，質的統合法（KJ法）を用いた分析・質的記述的分析等より，ツールの活用可能性について検証した。結果：看護師はツールの活用可能性について，【介護医療院という場で活用する意味】を認識する一方，【きっかけがない中で活用する難しさ】を認識していた。それに関連して，【高齢者の本音を聞く前段階としての活用】や【情報整理を進めるためのツールとしての活用】を模索し，これらが影響しあった結果，【同僚看護師への理解の深化】や【活用方法変更の見通し】，【新たな支援策の発案と継続活用の可能性】へと認識を広げていた。考察：看護師はツールを使用する中で，収集した高齢者の過去の背景に関する情報を個々に留まらせることなく，看護実践を共に実施するチーム内で共有することができていたと考えられ，本ツールは，看護師間で高齢者の過去の背景に関する情報を共有する一つの方法論として，臨床看護に貢献できるツールとして位置づけられると考えられた。

Aim: This study aimed to verify the possibility of utilizing an information sharing tool related to the history of older people. Method: Five nurses working at long-term care homes in the Tokyo metropolitan area utilized the information sharing tool related tor older people’s history (hereafter, tool) for 2 months, and focus group interviews were held before and after that utilization period. Thereafter, the possibility of utilizing the tool was verified through analysis using the qualitative synthesis method (KJ method) and Analysis of qualitative accounts of the nurses’ perceptions regarding the possibility of utilization following its use and aspects of individualized nursing care developed by using the tool. Results: While the nurses recognized the “impact of utilizing the tool in a long-term care home,” they perceived “difficulty in utilization amid a lack of opportunity.” In connection to that, they sought “utilization as a first step toward listening to older people’s true feelings” and “utilization as a tool for helping to organize information,” which consequently led to expanded awareness of “deepened understanding of nursing colleagues,” “ insight into changing utilization methods,” and “potential for proposing and continued use of new support measures.” Discussion: As the nurses utilized the tool, they were conceivably able to share information gathered on the history of older people within the team jointly carrying out nursing care, rather than such information being solely retained by individual nurses. This tool is thought to be capable of contributing to clinical nursing as a methodology for sharing information on older people’s histories among nurses.
