Abstract

A 76-year-old man had a liver tumor increasing in size on abdominal ultrasonography. Abdominal computed tomography (CT) showed a hypodense lesion on an unenhanced scan. Enhanced CT showed a ring-enhancing lesion in the arterial phase without washout in the portal venous phase. Contrast-enhanced ultrasonography showed a hyperdense lesion in both the arterial and portal venous phases but hypoenhancement in the Kupffer phase. Magnetic resonance imaging (MRI) showed high signal intensity on T1-weighted images and low signal intensity on T2- and diffusion-weighted images. On enhanced MRI, the lesion was hyperdense in the arterial phase and hypodense in the portal venous phase. Positron emission tomography CT indicated multiple areas with increased tracer uptake. No evidence of metastasis to other organs was observed. Histopathology revealed a primary hepatic leiomyosarcoma that was subsequently resected surgically. Five months postoperatively, the patient is alive without recurrence.
