Description

While it is regarded as important, there is little research documenting effective methods of professional development for mid-career nurses. Reflection on action and writing narratives are considered to be effective methods but specific effects are not clear, especially in Japan. The purposes of this study were : 1) to describe nurses' experiences who participated in the Career Development Ladder Programs, which, included as a component, reflective practice as expressed in nurses' narratives, 2) to find the necessary conditions, that make reflective practice based on nurses' narratives an effective professional development program.

In this qualitative study, data were collected from a convenience sample of 14 nurses, from 2 hospitals, using semi-structured interviews. Grounded theory analysis produced 12 categories and 38 sub-categories.

Described in this paper are the 3 categories. The core category was [Expansion of self-image] and its sub-categories included :〈Increased feeling of self-affirmation〉,〈Broadened outlook〉and〈Revised self-image〉. The theoretical model suggests that [Consciousness of nursing practice] and [Learning experience] through writing narratives and interaction with others during peer review and similar experiences brought abou [t Expansion of self-image], which led to [Changes in action]. Despite these effects, several subjects felt it made little change on them because of insufficient revision of their assumptions, values and beliefs.

To make the reflective practice effective, subject's reflections about their assumptions, values and beliefs and conditions which promote mutual support and trust in the peer review are essential.
