Abstract

Spontaneous rupture of the esophagus, which occurs as a result of a rapid increase in intra-esophageal pressure, can be fatal due to delay in the initiation of treatment. We report a case of spontaneous rupture of the esophagus successfully treated with laparoscopic surgery. A 52-year-old man was transported to our hospital with back pain and epigastric pain after vomiting. A chest and abdominal CT scan revealed fluid collection with air in the inferior mediastinum. We diagnosed spontaneous esophageal rupture, and performed emergency laparoscopic surgery after 12 hours from the onset. We confirmed a perforated site at the left wall of the abdominal esophagus and identified the abscess behind it. The mucosal layer of the perforated hole was closed by interrupted suture and muscular layer was closed by continuous suture, then covered with the omentum. The patient was discharged on the 12th postoperative day. Laparoscopic surgery is one of the effective options for spontaneous esophageal rupture even in an emergency situation.
