Treatment outcomes of 184 patients with parotid carcinoma

Abstract

This study investigated the clinical outcomes of patients with parotid carcinoma at a single institution during a 19-year period, with a focus on diagnosis, treatment, and survival. A review of the literature in Japan revealed no previous reports of over 100 cases with parotid carcinoma, so we studied 184 patients with parotid carcinoma treated at our department from September 1999 to February 2019. There were 22 patients in stage Ⅰ, 71 patients in stage Ⅱ, 23 patients in stage Ⅲ, and 68 patients in stage Ⅳ. Preoperative diagnosis of the histological grade was mainly diagnosed by fine-needle aspiration cytology and frozen section biopsy. In the surgery, preservation or sacrifice of the facial nerve was decided according to the pathological grade and the location of the tumor. The disease-specific 5-year survival rate was 100% for patients in stage Ⅰ, 97.7% in stage Ⅱ, 71.6% in stage Ⅲ, and 51.6% in stage Ⅳ. Multivariate analysis showed the pathological grade was the most important prognostic factor. According to our results and other reports, advanced and high-grade tumors had poor outcomes. Therefore, the development of new treatment strategies such as molecular-targeting therapies is required.
