Phyllodes tumors are uncommon fibroepithelial tumors that account for < 1% of all breast tumors, and the median age at diagnosis is 45 years. We report a case of borderline phyllodes tumor in an 18-year-old woman who presented to our hospital with a 10-cm-sized tumor in the right breast. The tumor gradually enlarged since she was 12 years old. Upon palpation, a 10 × 9-cm-sized tumor was found to be elastic hard and mobile. Ultrasound revealed an oval, well-circumscribed, hypoechoic mass. Differentiating a phyllodes tumor from a fibroadenoma by core needle biopsy is difficult, and the tumor was diagnosed as a fibroepithelial tumor. With a preoperative diagnosis of giant fibroadenoma or phyllodes tumor, lumpectomy was performed under general anesthesia. The pathological diagnosis was borderline phyllodes tumor with a negative surgical margin (0.1 mm), and the recommended treatment was wide excision with a margin of > 1 cm. However, good cosmetic outcome and preservation of breast-feeding ability are important for young female patients. Therefore, close clinical follow-up without re-operation for wider excision could be an option.
