Primary Small Bowel Cancer Initially Presenting as a Metastatic Carcinoma to the Testis

Abstract

Metastatic testicular cancers are rare ; the most common primary cancer is gastrointestinal cancer, but primary small bowel cancer is extremely rare. Cases of primary small bowel cancer initially presenting as a metastatic carcinoma to the testis have not often been reported in Japan. Here, we report a rare case along with a review of the relevant literature. A 70-year-old man presented with right scrotal swelling and was diagnosed with right-sided testicular cancer ; right-sided orchiectomy was performed by a urologist. Pathological examination revealed adenocarcinoma ; he was referred to our hospital for detailed examination. Radiographic examination revealed peritoneal dissemination and small bowel tumors. He underwent partial resection of the small bowel because of presence of small bowel tumors in the ileus. He was diagnosed with primary small bowel cancer and metastatic testicular cancer based on this pathological examination. mFOLFOX therapy was administered for those residual lesions. Local recurrence and peritoneal dissemination were not observed 1-year after partial resection of the small bowel, and he continued this chemotherapy.
