Scales for Postoperative Delirium in the Elderly : A Literature Review

<p>We conducted a review to identify recent trends and clarify issues concerning scales for postoperative delirium in the elderly following publication of the Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition, in 2013. A literature search was performed using Japan Medical Abstracts Society and PubMed databases, as follows : (“scale” OR “measurement”) AND “postoperative” AND “delirium.” The filter “aged : 65+ years” was applied. Through the database search, 136 studies were identified. Based on the selected inclusion and exclusion criteria, 31 studies were included (9 in Japanese, 22 in English). We identified 12 scales of delirium and 11 other scales for predicting delirium in this review. The Confusion Assessment Method was used in 11 studies and the Confusion Assessment Method for the Intensive Care Unit and the NEECHAM Confusion Scale were used in 5 studies, along with 9 other scales of delirium. Other scales for predicting delirium included the Comprehensive Geriatric Assessment and Mini-Mental State Examination, which were found in 2 studies each, as well as 9 others. A cohort study design was frequently used, appearing in 24 studies. The average number of participants was 134.2, and in 15 studies, they often underwent orthopedic surgery. They were evaluated one to three times per day. Nurse was the most frequent type of evaluator in 18 studies ; observation was the most frequent evaluation method in 19 studies.</p>
