Description

An 87-year-old man was admitted to our hospital with the complaints of swelling and pain in the right inguinal region. Abdominal computed tomography suggested herniation of the appendix through the femoral ring. We suspected De Garengeot hernia and decided to perform emergency surgery. We adopted a laparoscopic approach and confirmed the hernia orifice. The appendix was incarcerated in the femoral ring, and the patient was diagnosed as having De Garengeot hernia. The incarceration was difficult to release through the laparoscopic approach; therefore, we divided the appendix at the root and removed it using an anterior approach. This technique is useful for preventing contamination. We report that this hybrid surgical technique was very useful for incarcerated De Garengeot hernia.
