A case of severe hypothyroidism followed by transcatheter arterial chemoembolization with miriplatin-lipiodol suspension for the treatment of hepatocellular carcinoma

Abstract

<p>An 80-year-old male patient was diagnosed with hepatocellular carcinoma during follow-up after achieving a sustained virological response on interferon therapy for chronic hepatitis C. The tumor size was approximately 36 mm in diameter. We performed transcatheter arterial chemoembolization (TACE) with miriplatin-lipiodol suspension and gelation sponge material as treatment. Near-complete lipiodol retention was observed after TACE. However, the patient was readmitted due to dyspnea and anasarca on day 41 after TACE. Further investigation revealed myxedema caused by severe hypothyroidism. Tests for both anti-thyroid peroxidase antibody and anti-thyroglobulin antibody were negative. We started treatment with levothyroxine and gradually increased the dosage, after which FT4, FT3, and TSH levels gradually normalized. Thereafter, the patient's symptoms gradually improved. Therefore, administration of miriplatin suspension containing iodine may affect the development of hypothyroidism through the Wolff-Chaikoff effect.</p>
