<p>症例は74歳，女性．肺MAC（Mycobacterium avium complex）症のため，3剤治療中であったが，症状や画像所見は徐々に悪化していた．喀痰よりノカルジア菌が検出され，肺ノカルジア症と診断し，ST合剤（sulfamethoxazole/trimethoprim，スルファメトキサゾール/トリメトプリム配合剤）を開始し，治癒した．肺ノカルジア症は，免疫能が正常でも発症し得る．特に気管支拡張症との関連が示唆され，肺MAC症も注意すべき疾患の1つである．肺ノカルジア症に特異的な症状はないため，診断は患者背景から疑うことが重要である．</p>
