<p>A 38-year-old man presented complaining of abdominal pain. Acute appendicitis and generalized peritonitis were diagnosed, and appendectomy was performed the same day. Postoperatively, the patient was placed on a cefmetazole (CMZ) intravenous infusion. Gram-negative bacilli suspected to be anaerobic bacteria were detected from one of four anaerobic bottles comprising two sets of blood culture collected on presentation, and the strain was identified as Desulfovibrio sp. Sensitivity testing showed that it was resistant to CMZ, and the patient was switched to continued treatment with amoxicillin/clavulanate. Although only a few cases of Desulfovibrio sp. as the causative pathogen of enteritis, liver abscess, and bacteremia have been reported, it tends to be antibiotic-resistant and is difficult to detect on bacterial cultures, and for these and other reasons, it is a bacteria to be noted. A rare case of Desulfovibrio sp. detected from an appendicitis patient is presented as the first such reported case in Japan.</p>
