救命救急センター(以下,センター)は現在の東邦大学医療センター大森病院の3号館1階・2階で2004年より稼働し,急性期疾患を扱うことから内視鏡の緊急使用が多いため,内視鏡を自己所有しているが,これには十分な自己管理能力が必要となる.センターの診療体制は一部のスタッフや研修医が短期ローテーションで交代するということもあり,内視鏡の取り扱いが粗雑になるため2009年までは修理費が高額になる年もあり,管理体制は不安定であった.2010年6月よりチーム制を導入し減少傾向に転じたが2012年から増加傾向となった.2014年1月に内視鏡感染防御検討チームが発足,同年3月に管理マニュアルを作成,また10月からは内視鏡取り扱い従事者研修を開始,2015年4月には内視鏡および光源運用規定を作成した.その後の内視鏡修理費総額は2014年および2015年は劣化による修理費を含んでいるため増額してはいるが,1回あたりの修理費が減少していることから,重大な修理事象が少なくかつ人的要因による修理数も減少へと転じているといえる.自己所有内視鏡の管理にチーム制の導入・研修・マニュアル・運用規定の作成には効果があると考え,今後の動向を注意深く観察していきたい.Our critical care and emergency center of Toho University Omori Medical Center has four self-owned endoscopes for treatment for acute disease, and these devices require careful management. The Center is on the first and second floors of Building 3 and was opened in 2004. Endoscope use is complicated by the characteristics of our medical treatment system, which depends on the rotation of staff and medical interns. This complexity led to increasing costs for repair, until 2010. However, in June 2010 we established a team system for medical treatment, after which repair costs began to decrease. Repair costs started to increase again, in 2013, because the system expanded to include chronic care. An endoscopic infection prevention study team was established in January 2014. Development of management documentation was begun in March of that year, and worker endoscope management started in October. Regulations for the use of the endoscopes and light sources were developed in April 2015. Total repair costs were higher in 2014 and 2015 because they included repairs attributable to deterioration, but repair cost in relation to duration of use tended to increase. There have been few reports of serious damage, and the number of repairs attributable to human causes has also declined. The evidence suggests that introduction of the team system, training, and manuals and development of operation regulations has improved management of the owned endoscopes. We look forward to further analysis after collecting additional data.
