医療施設におけるアルコール以外の精神作用物質依存症(薬物依存症)患者への看護の歴史を明らかにし、今後の課題を考察することを目的に、医療・看護系の文献(1958年~2003年)を検討した。その結果、次のことが明らかになった。薬物依存症患者は看護上困難な事例として取り上げられていたが、系統的な事例検討は行われていなかった。また、薬物依存症患者への看護についての検討は、1960年代と近年に集中していた。それは、薬物関連問題への医療の役割が、社会的に要請された時期と重なっていた。検討内容の特徴としては、患者への看護者の感情反応が問題視される傾向にあった。そして、時代とともに事例の問題は複雑化する傾向にあり、地域との連携も着眼されていた。今後は、薬物関連問題の社会的状況をふまえ、看護の立場でも系統的な研究・実践を行う必要がある。特に看護者の薬物依存症患者への感情反応とケアに関する検討は課題と考える。

The purpose of this study was to review hospital nursing for drug dependent patients (inpatients) and to discuss future challenges. Medical and nursing articles in the current literature (1958-2003) were reviewed. While drug dependent patients are often considered by nursing to be “difficult,” these patients have not been studied systematically. Most of the nursing studies on drug dependent patients occurred in the 1960s. These were times when extensive medical services were required to meet the social needs of drug related problems. Many articles consider the emotional response of nurses to drug dependent patients a problem. In future, nurses will be required to understand the social problems related to improper drug use and study them systematically. Studies on the emotional response of nurses to drug dependent patients and its influence on patient care are of special importance.
