This study aimed to investigate the status of end-of-life care performed by nursing and care-giving staff in a nursing care home, and to clarify the issues with each profession that need to be solved in caring for end-of-life patients. Survey method: We conducted a self-administered survey with 400 nurses and 660 caregivers who were working at 200 nation-wide nursing care homes. The survey questions were based on the 33 items in the 8 practical areas of the end-of-life nursing care guidelines stipulated by the Health, Labour and Welfare Ministry. According to the results of the analysis, nurses showed significantly higher achievements than caregivers in five of the items. Caregivers showed lower achievements in the items where judgments or techniques that are based on medical knowledge were required. Furthermore, although there were no significant differences, both of the groups showed low achievements in the item “bereavement care for the family”. In addition to the education and training for both professions, the establishment and implementation of bereavement care protocols, as well as the development of a new work system that enables such protocols will be necessary in each facility. Furthermore, implementation of bereavement care for family members should be additionally included in the planning of requirements for end-of-life nursing care; thus, a review of the social system is also required.
