Abstract

For over 90% of patients who undergo catheter interventions for acute myocardial infarction (AMI), patient management takes the form of discharge from the hospital followed up with out-patient visits. On April 2010, a newly designed Iwate Chubu regional collaboration path for patient management following catheter interventions for AMI was introduced between two central hospitals and family clinic doctors. One and half years have passed since the introduction of the path, and 27 cases have arisen. Clinical courses, questionnaires, laboratory data, and coronary arterial image data obtained at 6 months follow-up were analyzed in 12 cases.

There was no recurrence of AMI. Medications had been well controlled. Followup catheter examination was done in all cases, as scheduled. One case of restenosis and one case of a new stenotic lesion of the coronary artery were observed. Serum LDL-Cholesterol and plasma glucose levels were significantly decreased.

Coronary risk management was well taken care of, and it can be concluded that improvement in the outcome after AMI can be expected where this regional collaboration path is introduced.
