Abstract

Verrucous carcinoma arising from an odontogenic cyst is extremely rare. We report a case of radicular cyst exhibiting verrucous proliferation of the lining epithelium, which resembles verrucous carcinoma of the oral mucosa. Panoramic radiographs showed a well-defined radiolucent lesion surrounding the root of the left maxillary first molar. The clinical diagnosis was radicular cyst. The cyst was surgically removed, and the left maxillary first molar was extracted. Histopathological examination revealed a cystic structure consisting of dense fibrous connective tissue, and most of the luminal surface was covered with stratified squamous epithelium with marked thickening and hyperkeratosis. The squamous epithelium had an acute angular spire-like shape in some areas, while the basal layer showed nuclear enlargement and nuclear fission, but the atypia was not so obvious and there was no obvious invasive growth into the wall. The lesion was diagnosed as a radicular cyst exhibiting verrucous proliferation of the lining epithelium. No recurrence and metastasis have been observed to date.
