Purpose: The aims of the present study were to develop the Japanese version of the Readiness for Hospital Discharge Scale (RHDS-J), which measures patientsʼ perception of readiness for discharge, and to establish its validity and reliability.

Methods: The RHDS was translated from English into Japanese by specialists of the science of nursing and the Japanese version was back-translated into English. After obtaining permission from the original publisher to ensure that the concept was identical with that of the original RHDS, it was adopted as the RHDS-J. Ninety-one adult medical-surgical patients who required medical support after discharge to home and received discharge teaching by nurses, were asked to fill out the RHDS-J 19 hours before discharge and other questionnaire including other variables that were needed to verify concurrent validity on the day before discharge and one week postdischarge.

Results: Factor analysis showed that the RHDS-J formed the same 4 subscales as the original RHDS: Personal Status, Knowledge, Coping Ability, and Expected Support. The goodness of fit was assessed using confirmatory factor analysis, and the model of the structure that was the same as that of the original RHDS was validated with the values. Cronbachʼs alpha for RHDS-J ranged from 0.78 to 0.94, supporting its reliability by showing internal consistency. Additionally, patients who had a high score on the RHDS-J had higher health-related QOL and self-efficacy on health behavior, more social support, and less anxiety, difficulties and deterioration of health condition at postdischarge.

Conclusion: The reliability, concurrent validity and predictive validity of the RHDS-J were confirmed. These results showed that the RHDS- J can be used to measure the readiness for hospital discharge of patients.
