Abstract
<p> We report a pregnant woman with hydronephrosis that bled inside during pregnancy. The patient had been diagnosed with a giant right hydronephrosis five years before pregnancy. She became pregnant at the age of 38, and her pregnancy course was uneventful until 23 weeks of pregnancy. She had a sudden onset of right frank pain and hematuria at 23 weeks of pregnancy due to a suspicious bleeding from the lining wall of expanded pelvicalyceal system, which stopped spontaneously in a week. The hydronephrosis, however, enlarged again to 32 x 22 x 18 cm in the next four weeks, and she began to suffer from abdominal distension and appetite loss. We aspirated the hydronephrosis by transabdominal needle puncture three times at 31, 34, 38 weeks of pregnancy, which relieved her symptoms. Just after the third aspiration, she delivered vaginally a healthy boy at 38 weeks of pregnancy. Repetitive aspiration may be a choice of conservative management for giant hydronephrosis of the mother during pregnancy.</p>
