Abstract

<p>59 歳，女性。初診の 9 年前より前頭部右側に徐々に増大する硬化性局面を認めた。限局性強皮症と考えられ経過観察されていたが，同部位に潰瘍を生じ皮膚生検にて基底細胞癌と診断された。下床の頭蓋骨を含めて広範囲切除し，前外側大腿遊離皮弁による再建術を施行した。病理組織学的所見では斑状強皮症型／破壊型であり，頭蓋骨内部への浸潤が認められた。切除断端陰性であったが，約半年後に第 5 胸椎左横突起転移を生じ切除術を施行した。その約1 年後右大腿骨内側顆に転移が出現し，切除・人工関節置換術後にシスプラチン・ドキソルビシン療法を施行した。その約 2 年後，右肺門部転移が出現しカルボプラチン・パクリタキセル療法を施行した。その後，特発性器質化肺炎を生じたため化学療法を中止し，プレドニゾロン投与にて加療した。その後は化学療法の再開を希望されなかったため，緩和的放射線療法を施行した。</p>
