<p>遺伝性血管性浮腫(HAE)は,身体機能障害をもたらす稀少疾患で,早期診断と効果的な治療が重要である.本稿は,HAEの診断と管理に関するWAO/EAACIの国際ガイドラインの改定と更新により,HAEの管理に関する最新のガイダンスを提供する.国際専門家委員会は,本ガイドラインの更新と改定のために既存のエビデンスを検討し,28の推奨事項を作成してオンラインのDELPHIプロセスによってコンセンサスを確立した.これらの推奨事項とガイドラインの目的は,医師とその患者が,C1インヒビターの欠損によるHAE(タイプ1)および機能不全のC1インヒビターによるHAE(タイプ2)の管理において以下のような共通かつ重要な臨床的問題に関する指針を提供し,合理的な決定を下すのに役立つことである.(1)HAEはどのように診断されるべきか?(2)HAE患者はオンデマンド治療に加えていつ予防薬を受けるべきか?また,どのような治療法を使用すべきか?(3)治療のゴールは何か?(4)子供や妊娠中/授乳中の女性などの特定のHAE患者グループではHAEの管理を変える必要があるか?(5)HAE患者は,どのように疾患の活動性,影響,およびコントロールをモニターすべきか? また,本ガイドラインは,HAEの管理に関する国際基準の確立を支援し,すべての患者に対して推奨される診断と治療の使用を奨励および促進することを目的としている.</p>
