Abstract

<p>38 歳，女性。妊娠 26 週，初産婦。3 カ月前より右大陰唇の皮下硬結を自覚し，増大傾向を認めたため，前医を受診した。右大陰唇に約 2 cm の弾性硬で圧痛を伴う皮下硬結を認め，超音波では血流豊富な囊腫様病変であった。生検で類上皮肉腫を疑われ，精査加療目的に当院を紹介され受診した。初診時，右大陰唇に半小指頭大の皮下硬結の残存を認めた。産婦人科と相談し検査を進めた。MRI では右大陰唇に T2WI・STIR・DWI で高信号，T1WI で低信号の皮下病変を認めた。深部への浸潤は認めなかった。単純 CT でリンパ節転移，遠隔転移は認めなかった。腰椎麻酔下に 2 cm マージンで残存病変の拡大切除術を施行した。センチネルリンパ節生検は施行しなかった。病理組織学的には真皮から皮下組織にかけての豊富な細胞質をもつ大型の類円形異型細胞の密な増殖を認めた。免疫組織化学染色にて INI1/BAF47 陰性であり，類上皮肉腫と診断した。遺伝子検査では SMARCB1 の欠失を認めた。切除断端は陰性であった。術後 3 カ月で経腟分娩にて出産し，術後補助療法は施行せず経過観察しているが，術後 19 カ月間再発は認めていない。</p>
