<p>Introduction: A patient of type 2 diabetes mellitus with lumbar canal stenosis and intermittent claudication due to symmetrical polyneuropathy developing as a complication of diabetes underwent minimally invasive spinal cord stimulation (SCS) under local anesthesia. We report interesting findings obtained of electrophysiological examinations, such as the central motor conduction time and cauda equina conduction time, which allow objective evaluation of spinal cord function.</p><p>Case: A 62-year-old man presented with low back pain, bilateral leg pain, intermittent claudication, and paresthesia of the feet. Magnetic resonance imaging (MRI) revealed 2–L3 lumber disc prolapse and lumbar canal stenosis. Electromyography (EMG) showed prolonged central motor and cauda equina conduction times. Therefore, corticospinal tract and cauda equina disorders were suspected. Three months after the SCS procedure, pain relief and improvement in motor/sensory functions were observed; his gait disturbance also improved, with a tendency toward recovery of the central motor and caudal conduction times.</p><p>Discussion: The patient's symptoms improved with SCS without invasive decompression of the lesion, suggesting activation and plasticity of the spinal cord neurons.</p><p>Conclusions: SCS, and electrophysiological examinations that allow objective evaluation of the spinal cord functions may be beneficial in diabetic patients with lumbar canal stenosis.</p>
