Abstract

A 50-year-old female had suffered from rough nails and intense itch on all her digits for about two years. When she first visited us, numerous longitudinal ridges were observed on the nail surface, resulting in rough and lusterless nails. In addition, the skin of the proximal nail folds was reddish and swollen with severe itching. A biopsy taken from the nail matrix histologically showed spongiosis of the matrix epithelium. Together with the characteristic nail changes, this case was diagnosed with idiopathic trachyonychia. Topical corticosteroids using the occlusive dressing technique resulted in marked improvement of the nail symptoms. We would like to introduce the disease entity of trachyonychia, which is not well recognized in Japan, and demonstrate the evaluation and management strategies for treating it.
