Abstract

Introduction : Methomyl is a carbamate insecticide that acts by reversibly inhibiting the enzyme acetylcholinesterase. While most carbamates are considered to have a relatively low toxicity, methomyl poisoning suppresses blood circulation and has a high mortality rate.

Case : An 88-year-old male presented with cardiopulmonary arrest, due to severe circulatory failure, after consuming methomyl in an attempt at suicide. Return of spontaneous circulation was noted after arrival at the hospital. Circulatory failure persisted, and the patient died of multiple organ failure 100 hours after admission.

Discussion : Organophosphates and carbamates are known to induce cholinergic syndrome. The combination of methomyl poisoning and post cardiac arrest syndrome may have contributed to the severe course of this case.

Conclusion : We report a case of methomyl poisoning with a serious clinical course.
