Abstract

We report our experience of simultaneous robot-assisted laparoscopic surgery for prostate and rectal cancer. The patient was an 80-year-old male. First, the surgeon performed sigmoid colon mobilization and lymph node dissection. While confirming the tumor site with a colonoscope, the rectum was transected on the distal side. Then, the urologist dissected open the Retzius space. A prostatectomy and vesicourethral anastomosis were performed next. A Rocco stitch was used as a one-layer suture between the dorsal urethral Denovilliers fascia and the dorsal bladder longitudinal muscle fascicle. The camera port site was extended, and the prostate was removed. After confirming the blood flow, the surgeon extracted the rectal stump and transected the intestine. A laparoscopy was performed again, and the intestine was anastomosed end-to-end. Two months after the operation, there was no incontinence, the maximum detrusor pressure was 48 cmH2O, and the anal maximum resting pressure/maximum squeeze pressure was 20/265 mmHg. One and a half years postoperatively, the patient’s progress has been good, with no recurrence, no fecal or urinary incontinence, and 16 mL post-void residual urine.
