Description

Purpose and Methods: Prenatal diagnosis of vasa previa is important. We conducted a literature search to extract reported cases of vasa previa in Japan since 2002. We reviewed 195 cases for which information was available, along with 9 cases from our own experience, making a total of 204 cases. We examined the optimal timing for delivery based on gestational age at delivery and outcomes.

Results: Of the cases where the timing of delivery was identified, 200 cases were prenatally diagnosed and underwent elective cesarean section, while 80 cases were either undiagnosed or diagnosed but required emergency cesarean section. The gestational ages at delivery for these 200 cases were as follows: less than 33 weeks in 10.5% of cases, less than 34 weeks in 16.0%, and less than 35 weeks in 32.0%. Excluding cases where cesarean section was scheduled before each respective week, the proportion of pregnancies continuing up to 33 weeks was 89.5%, up to 34 weeks was 84.4%, up to 35 weeks was 76.0%, and up to 36 weeks was 60.8%.

Discussion: Considering the proportion of pregnancies that continued, it was deemed sufficiently acceptable to wait until 34 weeks for delivery in cases of vasa previa.
