<p>A 65-year-old woman visited her local doctor with a complaint of persistent bloody stools. She was diagnosed with rectosigmoid colon cancer by colonoscopy. Preoperative computed tomography showed a horseshoe kidney and an extra renal artery branching from the left common iliac artery. Based on the preoperative diagnosis of rectosigmoid cancer (cT3N0M0, cStage II a), laparoscopic high anterior resection with D3 lymphadenectomy was performed. An ICG-fluorescent ureteral stent was also inserted in the left ureter preoperatively, and it was visualized intraoperatively. It was possible to perform the operation safely by maintaining an appropriate retroperitoneal detachment layer. Patients with a horseshoe kidney may show abnormalities in vasculature, the ureter, and autonomic nervous system, so careful preoperative examination and devices for safe surgery are imperative.</p>
