<p>79 歳，女性。初診 3 年前に背部にそう痒を伴う紅褐色斑が出現し，精査加療目的に当科を紹介され受診した。左側中背部に約 35×36 mm の境界明瞭な紅褐色斑を認め，病理組織では表皮内に胞体の明るい腫瘍細胞が増殖し，腺管構造やアポクリン分泌像を認めた。免疫組織化学染色では CK7，GCDFP-15，GATA3，mammaglobin が陽性で，CK20 は陰性だった。異所性乳房外パジェット病と診断し，10 mm マージンで摘出し全層植皮術を施行した。術後 19 カ月が経過し，現在再発なく経過している。</p>
