Abstract

The following case demonstrates the application of endosseous implants in the bone grafted clefts in combination with orthodontic treatment in a female patient with malocclusion due to bilateral complete cleft lip and alveolus. The patient was a girl, who presented with anterior cross-bite. Active treatment was initiated at the , and efforts were made so to align the bilatelal upper first incisors. After maxillaly expansion as well as extraction of the upper left lateral incisor were performed, autogenous particulate marrow and cancellous bone grafting from the iliac crest was performed in both clefts at the , and the upper right lateral incisor was removed simultaneously. Four months later, orthodontic movement of the permanent teeth was initiated using a multibracket method. After 10 months of active treatment, retention was performed for 16 months. At the , endosseous dental implants were inserted in the bony bridge. The period between the secondary bone grafting and implantation was 24 months. At the , the patient's occlusion remains stable. It was found that this method can be an option for treating malocclusion without orthdontic closure in patients with bilateral alveolar clefts presenting with congential missing of upper lateral incisors or those who have upper lateral incisors that are not involved in occlusion.
