The authors investigated direct bonding systems that were used in actual clinical practice in Japan. Our questionnaire was sent to 32 orthodontic departments of dental and medical schools and 23 private orthodontic offices in Japan. Forty-five replies (82%) were received. The questionnaire asked the following : 1) What type of adhesive system and what type of bracket are used in your clinic? What are the criteria of their selections? 2) What types of problems might occur when using orthodontic adhesives and brackets? 3) How can the materials and the methods be improved to overcome these problems? The findings were as follows : 1) Unfilled MMA-based adhesive (51.1%) and filled bis-GMA-based adhesive (48.9%) were used most frequently. Orthodontists chose bonding materials based on bond strength, handling properties, familiarity of the material and easiness of debonding. 2) Various problems with materials were reported and they varied between orthodontists. Two bracket-adhesive combinations that did not have enough bond strength for clinical use were reported. Three orthodontists reported the combination of ceramic bracket and filled bis-GMA-based adhesive and three others reported plastic bracket and light-curing adhesive. 3) For future bracket bonding systems, higher bond strength, easy handling, easy debonding, no enamel damaging, and bonding site enamel fortifying against decalcification were hoped for.
