矯正歯科治療後における患者および保護者へのアンケート調査

近年における患者・保護者の意識を把握し,矯正治療に対する認識と問題点を調査するためにアンケート調査を行った.2008年7月から2009年3月の9か月間,歯科大学の矯正歯科で動的治療を終了した患者121名とその保護者48名を対象として,以下の結果を得た.1.矯正治療終了時期と期間は,高校生までに終了を希望しているものが多く,短い治療期間を望んでいた.2.矯正治療を受けることに関しては,「自分の歯並びが悪かったので仕方ない」と感じる患者が半数を占めていた.治療中の評価では,「途中でやめたい」と感じた患者は全体の約1/3であり,その理由として「装置の不快感」,「治療の長期化」が多かった.また,装置の外観を「恥ずかしい」と感じた患者は,成人の金属ブラケット使用者でも17%と低く,矯正治療がより一般化している傾向がみられた.3.治療後の評価では,歯並びに満足した患者・保護者は有意に多く,矯正治療を受けてよかったと回答した患者・保護者は90%を超え,治療の効果が認められた.治療後は歯ブラシの習慣の改善に加え,成人では矯正治療後に自分の性格や習慣に変化か生じたと考えており,心理面での効果も認められた.4.他人の歯並びが気になると答えた患者は半数以下であり,他人の歯並びへの関心は低かった.将来,自分の子供に矯正治療を受けさせると考える患者は70%以上と多く,治療の必要性や効果が評価された.以上の結果より,矯正治療について患者・保護者の意識や要望が把握でき,今後の矯正治療の対応について多くの示唆が得られた.
The purpose of this study was to investigate the current opinions regarding post-orthodontic patients and their guardians. A questionnaire for collecting demographic and behavior variables was used in this study. Samples of 121 patients under observation at a dental university hospital and 48 of their guardians were selected sequentially from July 2008 to March 2009. 1. Many patients would like to finish treatment before completing high school and if possible, earlier. 2. As regards to orthodontic care, half the participants felt there was no choice for receiving treatment. One third of patients wished for discontinuation during treatment. Appliance discomfort and prolongation of treatment were indicated as the main reasons. According to adult patients using pliances, indicating that orthodontic treatment has become widely accepted in Japanese society. 3. More than ninety percent of the patients and guardians were satisfied with treatment according to post-orthodontic assessment. They changed not only their toothbrushing behavior but also their personality and attitude, which may affect their mental state. 4. Less than half the patients paid attention to others' alignment of teeth. However, >70% of patients considered supporting their children if they required orthodontic treatment, indicating that their orthodontic history led to a successful experience. These results reveal how orthodontic patients and their guardians feel about treatment; questionnaire surveys may contribute to improvement of practice.
