Description
We performed a study to clarify the hypothesis that poor uterine blood flow is a cause of failure to implant transferred embryos. The uterine arterial pulsatility indices (PI) were measured by transvaginal color Doppler sonography one day before embryo transfer (ET) in 41 infertile women who underwent in vitro fertilization (IVF) treatment. The serum estradiol and progesterone concentrations were also determined on the day of PI measurement. The mean PI values (±SD) were 2.90±0.74 in patients who conceived (n=14) and 3.43±0.60 in those who failed to conceive (n=27), and there was a significant difference between the values (p<0.01). When these patients were categorized into the low PI (<3.0) or high PI (>3.0) groups, a significantly higher pregnancy rate was observed in the low PI group (64% versus 19%, p<0.01). There was a negative correlation between the serum estradiol concentrations and uterine PI values (r=-0.44, p<0.05), whereas serum progesterone levels have no significant correlation. In conclusion, our data support the hypothesis that poor uterine blood flow is a cause of implantation failure in IVF treatment, and the uterine PI value is considered to be a useful indicator of uterine receptivity.
