Abstract
Septic shock is a life-threatening complication of serious Gram-negative bacteremia. Endotoxin plays an important role in the pathogenesis of septic shock. Many investigators have reported that polymyxin B-immobilized polystylene fiber (PMX-F) treatment is safe and effective in patients with septic shock. PMX adsorbed activated neutrophils and monocytes and reduced various inflammatory markers, adhesion molecules, urinary markers such as liver-type fatty acid binding protein and cardiac injury markers such as Troponin T and brain natriuretic peptide. In this review, we discuss 1. Recent report of septic shock itself, 2. Clinical research about PMX-F on septic shock, 3. Effect of PMX-F on respiratory diseases such as pneumocystis pneumonia, rapidly progressive interstitial pneumonia and influenza A infection, 4. Recent case report. We discuss the importance of advanced glycation end product (AGE) and receptor for AGE on septic shock and adult respiratory distress syndrome and the effect of PMX-F treatment.
