Suppose that a decision maker having a problem to be settled under assistance of a MS/OR professional is not good at understanding mathematical models. Then, preparing a document (problem specification) of a problem formulated into a model is useful for their mutual communication. GERM (generic ERM) is a format of problems specification proposed by the authors' group and is an extension of ERM (entity-relationship model). This paper proposes a procedure for preparation of GERM problem specifications. It is useful when no existing problem specification is available for the current world of discourse.
