症例は2歳の男児.右腹部に巨大な腫瘤を触知.血清AFP値186,699ng/ml, CTで腫瘍は右葉,尾状葉内に位置し下大静脈を取り巻き,肝芽腫と診断された(PRETEXT IV).骨髄移植を含む大量化学療法も奏効せず,遠隔転移はないが切除は不可能と判断され,Primaryに生体肝移植を行った.奇静脈系の側副血行路が十分発達し,下大静脈を合併切除した.術後AFP値は順調に低下し再発・転移の兆候はない.化学療法後にも切除不能で,遠隔転移の無い肝芽腫では,Primary肝移植がよい適応で,生体移植は治療を計画的に行える点で優れている.
