Description

The growing numbers of childhood cancer survivors necessitates a long-term follow up after the completion of cancer therapy. Hence, generating constructive and instrumental systems for long-term follow up in these patients is the need of the hour. The Japan Pediatric Leukemia-Lymphoma Study Group (JPLSG) developed a format titled as the "Summary about Cancer Treatment" for systematic follow up. The authors expect this summary to play an important role in terms of the situation of the survivors' movement, shift in the physicians in charge, or the survivors' transition to an adult clinic. The authors have discussed the application and significance of this summary.
