Although Granulocyte and Monocyte Adsorptive Apheresis (GCAP) is an effective therapy with very few side effects for patients with ulcerative colitis, there still remains some insufficiency with regards to its efficacy. We attempted to improve the efficacy of GCAP, by determining the predictors of the outcome in addition to enforcing treatment sessions of twice a week instead of once a week, in order to shorten the days taken until remission. These steps can result in the induction of remission with GCAP in patients with active ulcerative colitis without steroid administration, thus increasing the potential for GCAP to become the first line therapy for remission induction in patients with ulcerative colitis. In addition, GCAP was shown to induce remission repeatedly with every relapse of the same patient. Thus, we concluded that GCAP has potential to become one of the standard therapies within UC remission-induction therapies. GCAP combined with a medium quantity of steroid can induce remission in intractable severe cases, with the exception of those with broad mucosal defect and fulminate conditions. A further study of the contribution of GCAP in the maintenance therapy for long-term remission has become a necessity.
