A Case of Verrucous Carcinoma Successfully Treated with Combined Radiotherapy and Carboplatin

Abstract

We treated a 76-year-old man who initially presented at a local hospital with a rapidly growing tumor at the left angle of the mouth that he had first noticed in November 1998. He was referred to our hospital on January 6, 1999, after being suspected as having a squamous cell carcinoma. At presentation, he had a cauliflower like tumor that was 4cm×5cm in diameter and 4cm high, was partially necrotic on the surface , and extended into the oral cavity.<BR>Histopathologically the epidermis resemble a verruca by showinghyperkeratosis, parakeratosis, and acanthosis. The keratinocytes appear well differentiaed, stain lightly with eosin, and posess a small nucleus. Nuclear atypia and individual cell keratinization are minimal or absent, and horn pearls are absent. The tumor invades with broad strands that contain pseudohorn cysts. From above findings, a diagnosis of verrucous carcinoma was made. We decided to first attempt to reduce the size of the tumor with combined radiotherapy and chemotherapy because the patient had severe obstructive pulmonary disease and general anesthesia therefore carried considerable risk. Treatment with radiation (total dose of 60 Gy) and carboplatin (total dose of 350mg) for approximatery two months resulted in a marked reduction in tumor size. There has been no evidence of tumor recurrence in the three years since discharge.
