Recurrent bladder leiomyoma with ovarian steroid hormone receptors

A 30-year-old woman (gravida 4, para 1) first presented in 1985 with a sensation of tumor on the vulva and difficulty voiding. Vaginal examination demonstrated a goose egg sized tumor on the anterior wall of the vagina and a descended uterus. Vaginal hysterectomy and tumorectomy were performed. Gross and microscopic examination of the specimen showed leiomyoma originating in the bladder or urethra. In July 1997 the patient returned, with pollakiuria, genital bleeding and a tumor on the vulva. Visual inspection of the vulva showed a fist sized, hemorrhagic, protruding tumor on the right side of the vaginal vestibule. The urethral orifice was displaced to the left side (fig. 1). Ultrasonography and magnetic resonance imaging showed a 6.3 3.8 cm. mass consisting of 2 nodules (fig. 2). The tumor was excised transvaginally. Histological examination revealed leiomyoma. Enzyme immunoassay demonstrated receptors for estradiol (56 fmol./mg. protein) and progesterone (310 fmol./mg. protein). Postoperative course was uneventful.
