We experienced one case of chronic alcoholism who had neuromyopathy and vitamin B_6 deficiency. Muscle and peripheral nerve biopsies were conducted twice, in the worsening period and in the recovery period. On the first biopsy, histopathological findings of the peripheral nerve showed remarkable changes of Schwann cells and the reduction of un-myelinated nerve fibres and particularly of thick myelinated fibres. Pathological findings of the skeletal muscle included neurogenic changes, disarrangement of myofibrils and mitochondrial abnormalities. On the second biopsy, the number of fine un-myelinated nerve fibre markedly increased. Skeletal muscle showed type 2A predominance and the reduction of type 2B fibre. These were considered as the results of peripheral nerve regeneration and the effect of rehabilitation. We discussed the possibility that vitamin B_6 deficiency might play an important role on the occurrence of neuromyopathies in chronic alcoholism patients.
