パラトグラムによる/s/・/t/・/k/各子音生成と上顎口蓋部形態形成 : 舌切除の有・無による調音の比較

日本語子音の生成に、舌と口蓋形態は重要な役割を果たしている.日本語母音・子音調音時の舌の口蓋への接触状態を示すパラトグラムは, 舌の機能と口蓋形態の両者を表現し, また発語者の舌の調音の固有性を表している.我々は, この間、日本語音節の生成に密接に関与するパラトグラムを用いて、舌が切除された患者あるいは健全な舌を持つ普通に話す無歯顎患者を対象に, 日本語子音を用いた上顎義歯の口蓋部形態形成法を提案し、それを用いて製作した上顎義歯により発音を含めた口腔機能の回復・改善に良好な成果を得て、その結果を報告してきた.そこで、今回は, 1)舌が健全で普通に話す上・下顎の無歯顎患者および2)舌が半側切除された上・下顎無歯顎患者を対象に, 日本語子音/s/・/t/・/k/のパラトグラムを用いた口蓋部の形態形成法の術式とその特徴を報告する。
The tongue and palatal shape play an important role in Japanese consonant soundproduction. Lingualpalate contact palatograms of Japanese vowel and consonant articulation express both tongue function and palatal shape as well as capture each patient's particular lingual articulation characteristics. A method for fabricating maxillary dentures with an augmented palatal design utilizing palatograms with a close relationship to speech articulation was developed through studying both glossectomy and non-glossectomy patients. Dentures created using this method have effectively improved general oral function, including sound production, in normal speaking and glossectomy patients. This study explores the techniques, using palatograms of Japanese consonants/s/・/t/・/k/ and characteristics of augmented palatal shape formulation in 1) Upper and Lower edentulous patients with healthy tongues, 2)Upper and Lower edentulous patients with half-glossectomized tongues.
