Description
We reported a 39-year-old man who was admitted to Kitasato University Hospital because of the cardiopulmonary arrest on 10, June 1991. An electocardiogram showed stand still. Soon after CPR, he developed extreme hypertension above 200 mmHg accompanied with excess sweating, muscle cramp, hyperpyrexia above 40℃, rhabdomyolysis, myoglobinemia and acute renal failure. His chest roentgenogram showed mild cardiomegaly with pulmonary edema. An electocardiogram after CPR showed ST depression and negative T waves in leads I, II, III, aVF, V_4, V_5, and V_6, poor-R wave progression in precordial leads. An echocardiogram revealed hypokinesis of anterior septal wall and the thickened lateral ventricular wall. The CT scan of the brain revealed no abnormalities, however, it were inactivity, record of electrocerebral. As an origin of his cardiopulmonary arrest, we suspected some intoxication because his symptoms were compatible with methamphetamine intoxication. Methamphetamine blood concentration at the first hospital day was 9.17 μmol/100 ml that was a fatal level. High methamphetamine level continued for several days. In this report, we discussed that one should consider intoxication as a cause of cardiopulmonary arrest in the case of unexplained clinical course and no abnormalities in routine blood examination.
