Based on cohort studies on a large scale conducted in various parts of the world in recent years, I investigated the relation of total cholesterol level to mortality rate. From the study by cause of death, it was found in all of the cohorts that the mortality rate due to coronary heart diseases almost rectilinearly rose as total mortality rate rose. Meanwhile in all of the cohort studies, the group with the lowest total cholesterol level showed high death risk of malignant tumor. As for stroke, the obvious relation with total cholesterol level could not be noted. Investigating the relation of total cholesterol level to total mortality rate, on the whole, the studies in Europe and North America showed a J-shaped curve which total mortality rate rises with a rise in cholesterol level, but the studies in Asia rather showed an inverse J-shaped curve which total mortality rate gently rises in the group with low cholesterol level. A difference like these patterns seems to have been caused by the structure of causes of death in both regions. For Japanese, the rise in total mortality rate with the rise in total cholesterol level was hardly found, so that it would not seem to be of great significance on the risk selection.
