Description

A case of primary biliary cirrhosis (PBC) with chronic thyroiditis was reported. A 59-year-old female who had had biopsy-proven "chronic active hepatitis" for ten years was admitted to Ushioda Hospital because of increased jaundice and ascites. Biopsy of the liver performed after treatment, however, disclosed PBC (stage II after Scheuer). Serological examinations were positive for antimitochondrial antibody, thyroid test and microsome test in contrast with negative for other autoimmune antibodies as well as hepatitis virus markers. Serum levels of immunoglobulin M, transaminases and thyroid stimulating hormone were all elevated, whereas those of T_3, T_4 and free T_4 were decreased. CT scan of thyroid gland demonstrated an appearance compatible with chronic thyroiditis (Hashimoto). She was successfully treated with glycyrrhizine (60〜80ml iv daily) and thyroid hormone with resultant improved laboratory abnormalities.
