Study on the Feasibility of G-CSF as a Supportive Drug in Ovulation Induction for Luteinized Unruptured Follicle

Introduction : The role of granulocyte colony-stimulating factor (G-CSF) in ovulation has been clarified in the last 10 years. In this study, I first examined the presence of G-CSF receptors and its mRNA in the follicles. Based on the studies that show important roles of G-CSF in ovulation, I performed a clinical trial to induce ovulation by G-CSF in patients with luteinized unruptured follicle (LUF). Materials & methods : 1) The presence of G-CSF receptor in the follicle was examined in 8 ovarian tissues immunohistochemically. The presence of G-CSF mRNA was also examined in 6 other follicular wall tissues by RT-PCR. 2) G-CSF 100mg was administered at 24±48 hours before hCG to 18 cycles in 16 patients whose last ovulation induction failed in LUF. Ovulation was then confirmed by ultrasonography. Results : 1) G-CSF receptor was detected in the granulosa cells and luteal cells in all follicles examined. G-CSF receptor mRNA was only detected in the follicle wall of the late follicular phase and ovulatory phase. 2) Ovulation was successfully induced in 16 cycles. The successful rate of ovulation induction with G-CSF was significantly higher than in the cycles without G-CSF (P<0.01). Conclusions : G-CSF receptor and its mRNA were detected in the follicle, and ovulation induction with G-CSF is an effective treatment for patients with LUF.
