A Case of Type I Cryoglobulinemia Probably Associated with Monoclonal Gammopathy of Undetermined Significance (MGUS)

Abstract

We experienced a type I cryoglobulinemia patient who had been treated for 16 years. A 68-year-old female had developed invasive purpura and ulcerative lesions on the face, and limbs with exacerbation in winter since 1991. An elevation of monoclonal IgG-κtype I cryoglobulin was detected by laboratory studies, and histopathologic analysis revealed the microvascular thrombus with eosinophilic amorphous materials in the dermis. Neither involvement of organopathy such as renal failure, or neuropathy, nor complications of malignancy like multiple myeloma were found. We diagnosed this patient as type I cryoglobulinemia associated with monoclonal gammopathy of undetermined significance. <BR>Although she had been treated well with antiplatelet drugs and systemic prednisolone during the course, sudden aggravation of skin symptoms was observed in April, 2007. After the various medical examinations, we concluded the aggravation was not because of the advancement of her cryoglobulinemia, but owing to her mental dysfunction associated with this disease.
