Description
The echocardiography, which has been rapidly developed as an important noninvasive technique during the past few years, has made it possible to observe the pathophysiological conditions of patients with ischemic heart disease. In this study, the following observations have been made : (1) Eehocardiographic examination was performed in 23 cases with acute myocardial infarction (AMI) and its serial changes were observed for 30 days after the admission to the hospital. (2) In 29 cases with old myocardial infarction (OMI), M mode echocardiographic observation were made. (3) Findings of echocardiography were compared with those of selective coronary angiography in 29 patients with OMI. Results obtained were as follows : (1) In cases with AMI, the abnormalites in left ventricular wall mortion at the infacted area were observed. In servived cases these abnormalities were gradually impioved. The value of AC/DC, which was known to reflect the left ventricular enddiastolic pressure (LVEDP) was decreased in accordance with the lapse time. The LVDd gradually increased, and the compensatory mechanism due to the extension of viable muscle was suggested. Thus. the observed parameters of AC/DC aod LVDd were considered as useful for evaluating the left ventricular function in AMI. (2) Various parameters of echocardiography in OMI indicated almost the same value as the values measured on the 30 th day in AMI. (3) It was, thus, considered that the latter value was of great use for asessing the prognosis of patients with myocardial infaction a good correlation was recognized between findings of coronary angiography and those of echocardiography. From these results, it is concluded that echocardiography is a very useful clinical and noninvasive examination for assessing the pathophysiological aspects in patients with acute and old myocardial infarction.
